One-lung anesthesia

چکیده

One-lung anesthesia was performed first by Gale and Waters in 1932.' They devised a curved, singlelumen cuffed tube which could be passed blindly into either mainstem bronchus.2 This accomplishment spurred the evolution of a variety of endobronchial tubes for one-lung anesthesia over the following years. One-lung anesthesia has been utilized for thoracic surgery for several decades. The lateral position is used most frequently, with the dependent or ventilated lung down. To enhance surgical exposure, a double-lumen endobronchial tube is inserted to allow the upper nondependent lung to be collapsed. Over the years, other indications have arisen for one-lung anesthesia that also have called for improvements in the technique. Today, nurse anesthetists must be aware of the indications for onelung anesthesia, the variety of endobronchial tubes available, methodology and basic pathophysiology, as well as possible complications and their management.

The safety of one, or repeated, vital capacity maneuvers during general anesthesia.

UNLABELLED A vital capacity maneuver (VCM) (inflating the lungs to 40 cm H(2)O for 15 s) is effective in relieving atelectasis during general anesthesia or after cardiopulmonary bypass (CPB). The study was undertaken to investigate the safety of one or repeated VCM. Five groups of six pigs were studied. Two groups had general anesthesia for 6 h and one group received a VCM every hour. Three oth...

Arterial oxygenation during one-lung ventilation: combined versus general anesthesia.

UNLABELLED The optimal anesthetic management of patients undergoing thoracotomy for pulmonary resection has not been definitely determined. We evaluated whether general i.v. anesthesia (propofol-fentanyl) provides superior PaO2 during one-lung ventilation (OLV) compared with thoracic epidural anesthesia (TEA) with supplemental local and general anesthetics. We studied 60 patients who had prolon...

Effect of desflurane-remifentanil vs. Propofol-remifentanil anesthesia on arterial oxygenation during one-lung ventilation for thoracoscopic surgery: a prospective randomized trial

BACKGROUND One-lung ventilation during thoracic surgery frequently disturbs normal systemic oxygenation. However, the effect of anesthetics on arterial oxygenation during one-lung ventilation has not been well established in human study. In this clinical trial, we investigated whether a difference between desflurane-remifentanil and propofol-remifentanil anesthesia can be observed with regard t...

Conductance of Anesthesia in Pediatrics Pulmonary Hydatid Cyst: A Case report

Background & Objectives: The most common parasitic lung disease is echinococcosis, and usually young male patients suffer from this disease. Lung involvement is more common in children, and it is important to consider this involvement as a diagnosis. Anesthesia and surgical management are always challenging for both Anesthesiologist and surgeon.  Case: In this case we report successful anesthet...
